Long Term Care Options Counseling

The program provides information and referral services related to out-of-home care facilities for older adults age 55 and older in Los Angeles County and nationwide. Services include long term care options counseling, nursing facility referrals, respite care registries and senior housing information and referral. Information about options for older adults with Alzheimer's Disease or other dementia is also available. There are no geographic restrictions.

The program connects individuals to local Senior Living Advisors, who conduct an assessment of the older adult's lifestyle, care needs, budget and preferences; and then provide information about senior living options. Advisors may also schedule tours of facilities, offer tips on meeting specific older adult needs, and provide support through the transition period.

The program maintains a comprehensive registry of senior out-of-home care service providers; providers pay a fee to the company for each successful placement. There are no fees for older adults or families to use this service.

Referrals to respite care providers is also available for family caregivers who need a break, or for families who are gradually easing their loved one into a life at a senior community, or as a trial of a senior community when a family is deciding whether it is the right option for them.
