Weight Management (Youth, Medicaid Recipients)

The agency provides weight management education and support services for Health Net's Medi-Cal and Healthy Families Program members, age 6 to 20. To be eligible, youth must be referred by a contracting Health Net Physician. There are no geographic restrictions.
Health Net members can access educational resources including a workbook, cookbook and nutrition & physical activity DVD. Some members may also speak with a live nurse in their perferred language to create a personalized nutrition plan, get weight management education and resource materials. Fitness and nutrition classes are also available to members and the community.
