General Paratransit/Community Ride Programs
Provider: CITY OF AZUSA PUBLIC WORKS DEPARTMENT - AZUSA DIAL-A-RIDE
The city provides transportation services for Azusa residents of any age who have a physical or mental disability and seniors 60 years and older, however people under 60 years of age must first be certified by Access Services or have a L.A.C.T.O.A. ID card to be eligible for service. Services include the Azusa Dial-A-Ride and the Azusa Metrolink shuttle. Dial-A-Ride services are restricted to Azusa residents.
The Dial-A-Ride program provides curb-to-curb transportation to any location within the city of Azusa and within a six mile radius of Azusa for medical and social services only. Vans are equipped with wheelchair lifts. Attendants and companions may accompany seniors and people who have a disability. Medical appointments are given priority.
The Metrolink Gold Line shuttle is offered on a demand response basis to residents and employees of Azusa businesses who commute by train to and from the Covina station. Prior reservations are required to schedule pick ups within the city.
The Foothill Transit Bus Pass program subsidizes bus passes for individuals with low incomes who reside within city limits.

Application Procedure
Call for Dial-A-Ride 3 days in advance; reservations are required. People younger than age 60 must have proof of physical or mental disability through certification by Access Services or have a L.A.C.T.O.A. ID card.
Foothill Transit Bus Pass assistance is available monthly on a first-come, first-served basis until subsidies are gone. Call or visit the Azusa Senior Center for service at 740 Dalton Ave.

Fees
The Dial-A-Ride program charges $ .50 to $1.50 per one-way trip per person depending on the destination. Fares are based on one-way trips only and paid to the driver.
The Metrolink Gold Line shuttle fare is $ .50 each way.
