Case/Care Management
Provider: CITY OF GLENDALE COMMUNITY SERVICES AND PARKS DEPARTMENT - SENIOR PROGRAMS - ADULT RECREATION CENTER
The program provides services for older adults who are 60 years and older. Services include case/care management and senior centers. Services are targeted to Glendale residents.
The Supportive Services Program (SSP) provides case management services to assist older adults live independently in the community. Staff visit the senior at home, assess the types of services the person could utilize, and link the senior with agencies that provide the services to allow more frail seniors to stay in their homes as long as appropriately possible.
