Transitional Mental Health Services
Provider: HILLVIEW VILLAGE - RESIDENTIAL SERVICES
The agency provides transitional and residential mental health services for people of all ages including transition age youth and ex-offenders in the San Fernando Valley area. Services include The service area is restricted to Arleta, Pacioma, Sunland and Tujunga and the surrounding communities.
The Full Service Partnership (FSP) program offers intensive, field-based psychiatric care and case management. Programs are divided by age group: ages 26 to 59, ages 59 and older, and transition age youth 16 to 25.
Ongoing services for individuals ready to transition back into their communities are provided through the Recover, Resilience and Reintegration (RRR) program.
Transition services are also available for ex-offenders upon release from incarceration.
