Disability Related Transportation
Provider: CITY OF CULVER CITY PARKS, RECREATION AND COMMUNITY SERVICES - CULVER CITY SENIOR CENTER
The center provides transportation services for older adults, age 50 and older, who live in Culver City and surrounding areas, including Windsor Hills, Ladera Heights, and View Park. Services include disability related transportation and local transit passes. There are geographic restrictions.
Dial-A-Ride and Taxi Voucher programs include disability related transportation provided for qualified individuals of any age who are transportationally disabled (unable to drive or use the public transportation system). Participants must be Culver City residents as defined by the Geographic Information System (GIS) and are required to complete a Paratransit Physician Authorization Form. Forms are available at the Senior Center or online. Once registered, participants must call one day in advance to request transportation. Common destinations include grocery stores, recreational facilities, beauty salons, doctor offices, etc. Priority is given to trips for medical appointments. All trips must take place within in the boundaries of Culver City with the exception of the Kaiser-Permanente medical facility located at 6041 Cadillac Ave.

Application Procedure
Call or walk in for information or to receive service. Disability related transportation requires documentation of disability on file. Taxi coupon books require a doctor's form on file.
Fees
Taxi coupon books cost $2 each. Taxi drivers are obligated to take up to $8.00 in taxi coupons per trip, over $8.00 must be paid out of pocket. Call taxi company to confirm current rates.
Dial-A-Ride has a suggested donation of $.50 per trip.
