The program provides health education services related to diabetes control in the San Fernando Valley area of Los Angeles. Services include community wellness programs that provide awareness and self-management assistance for diabetic patients and families; cooking classes and nutrition education in a special Diabetes Teaching Kitchen; disease/disability information on diabetes offered in a classroom setting and in the community. There are no geographic restrictions.

Project ALTO is a community wellness program in which Promotadores provide diabetes awareness and disease self-management outreach and classes for individuals and families. Classes include basic nutrition, goal setting, physical activity, information about medications, and disease self-management strategies to keep diabetes symptoms under control.

The Diabetes Teaching Kitchen provides culturally sensitive instruction in nutrition and food preparation to combat obesity. It provides hands-on demonstration classes that combine clinical nutrition information with food preparation techniques.
