Health Care Referrals (Native American Community)

The agency provides health services to Native Americans/Alaska Natives who live in Los Angeles County. Services include public health case/care management services, dental referral services, diabetes clinic services, health care referral services and psychiatric referral services. There are no geographic restrictions.
The program is a linkage referral service which provides comprehensive case management. Services provided include site visits, scheduling appointments, transportation and coverage of pre-approved costs, including pharmaceuticals if the pharmacy is attached to the referred agency.
The Los Angeles American Indian Community Clinic provides the following clinic services to registered clients - physical exams, school physicals, well-child services, preventive medicine, immunizations, women's health, laboratory services, STD/STI detection and treatment, HIV testing, diagnosis and management of acute and chronic diseases.

Application Procedure
Call to apply; it is recommeded that clients work with their case manager first before scheduling an appointment. All individuals must be registered with the agency to receive services. Clients must be registered with the agency. To be registered, clients must provide proof of American Indian lineage; this proof may be a CDIB (Certificate Degree of Indian Blood); as well as a Social Security card and CA I.D. card.
Fees
There are no fees for services. Dental, medical or mental health services provided to eligible clients are at no cost contingent on availability of funds.
