Case/Care Management (Homeless Families)

Provider: ILLUMINATION FOUNDATION

The agency provides case/care management through the Bringing Families Home program. Service includes housing retention, housing navigation, and case management to individuals experiencing homelessness or at risk of homelessness. There are no geographic restrictions.

The BRINGING FAMILIES HOME (BFH) program provides housing services to eligible homeless families who are currently receiving court ordered or voluntary family maintenance services, so the parents can maintain custody of the children in their home.

Application Procedure

For the BFH program homeless families must be referred by their social worker and contact the 211 BFH office on the 3rd floor of the Children's Court, Rm. 3005.
