Personal Care

The agency provides in-home health services for people of all ages who live in the East San Gabriel Valley, including the cities of San Dimas and La Verne, San Bernardino County and parts of Riverside County. Services include case/care management, health education, home health aide, home nursing, homemaker assistance, hospice care, mother and infant care, nutrition education, occupational therapy, physcial therapy, personal care, stroke rehabilitiation and volunteer opportunities. There are geographic restrictions.

Most services require a physician's authorization and supervision. The goal is to provide skilled, professional services for people who are ill or have a disability so that they can remain in their own homes.

Home health services available include skilled nursing; physical, enterostomal, occupational, and speech therapy; mother-infant care; I.V. therapy; home health aides; nutrition education; and medical social services. The skilled nursing assistance may include wound care, ostomy care, education for diabetics, and stroke rehabilitation. Pediatric nursing is also available. Home health aides assist with personal care.

Hospice services provides a team of professionals who assist patients and their families during end-of-life, allowing patients to remain independent and at home. The interdisciplinary team consists of physicians, RN's, social workers, chaplains, home health aides, volunteers and the family. Other skilled personnel may be part of the team, if needed, and trained volunteers support the patient and family. In addition to providing regularly scheduled home care, the team is available 24-hours per day for emergencies.
