Wraparound Facilitation/Community Support

The agency provides mental health services for youth, age 2 to 25, in Los Angeles County. Services include in-person crisis intervention, outpatient psychiatric services, developmental assessment, behavioral learning therapy, psychiatric medication monitoring, and wraparound facilitation/community support services. Age and income eligibility requirements apply for some programs. There are no geographic restrictions.

All services can be provided in the community and in the home. The agency also provides prevention and early intervention services.

Staff provide crisis intervention to help individuals cope with a crisis while maintaining them in their current community and level of functioning. This service is limited to stabilization; staff provide assessment, evaluation, medication monitoring, therapy and linkage to other programs.

The agency uses wraparound as an alternative to residential or group home institutional care. The goal is to keep the child at home while receiving treatment or if the child is already in institutional care, to return them home sooner. Wraparound is not a specific program or type of service but a process. Wraparound is conducted by a multi-disciplinary team consisting of a social worker case manager, a therapist and a parent "Partner". Sessions are conducted in the home and may involve just the child or the whole family as needed. The program accepts youth from age 5 to 22, and may continue for one year or longer. The service area is L A County SPAs 1, 2 , 3, and 7.

Medi-Cal eligible youth, 0 to 18 years of age, may receive developmental assessment services and behavior modification therapy through the CHDP program.

Behavioral learning therapy is provided for youth and young adults to help them replace target behaviors with adaptive behaviors, skills, and strategies. These services are generally provided in conjunction with other planned mental health services.
