Adult Day Health Programs
Provider: WESLEY HEALTH CENTERS ADULT DAY HEALTH CARE
The agency provides adult day health care for older adults, adults with disabilities, including adults with Alzheimer's Disease who live in the Antelope Valley, Lancaster, and Palmdale areas of Los Angeles County. Services include adult day health programs.
Participating adults must suffer from chronic health problems that affect their ability to perform normal daily activities; have a medical condition requiring rehabilitatitve therapy or treatment ordered by a physician; or require therapeutic recreation, socialization, and mental stimulation or observation on an ongoing basis.
To enter the program, a TB test is required. A pre-qualification screening is done, followed up by a home visit to assess and qualify the client for Medi-Cal coverage. Documentation requirements are Social Security card or Medi-Cal card; letter from referring physician; proof of age; proof of residence. A center-based assessment is then done that includes a health screening, and an evaluation by Occupational and Physical Therapists.
