Child Health and Disability Prevention Exams

The agency provides health services for low-income, uninsured, and underinsured families who live, primarily in the San Gabriel Valley. Services are targeted, but not restricted to the Chinese community. The clinic will serve undocumented immigrants, and will serve HMO patients. Services include adolescent/adult immunizations, alternative medicine, certificates/forms assistance, child health and disability prevention exams, childhood immunizations, community clinics, contract clinics, dental care, general physical examinations, mammograms, and ultrasound screening. Some low-cost programs require proof of California State residence and others require proof of Los Angeles County residence.

Contract clinic services include outpatient health services for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary general medical care. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients. When client lists are full, the Contract Clinic services to new clients are placed on hold and only ongoing patients can be seen.

Child health and disability prevention exam services provide regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.
