Long Term Case/Care Management (Medicaid Recipients, People With Complex Needs)

The agency provides case management services for adults in Los Angeles County. Services include long term case/care management and at risk/homeless housing related assistance programs for Medi-Cal recipients who have complex needs. Eligibility restrictions apply; there are no geographic restrictions.

Enhanced Care Management is a Medi-Cal benefit that provides eligible members with a lead care manager who coordinates their health and health-related care and services. Specific criteria apply.

The agency also provides housing related assistance for Medi-Cal members through its housing navigation program. Eligible adults include those who are:

  • experiencing homelessness and meet the HUD definition of homeless
  • at risk of becoming HUD homeless
  • exiting an institution, was HUD homeless prior to entering, and would become homeless again immediately upon release
  • meet the HUD definition of chronic homelessness
  • additional eligibility criteria may apply.
