PACE Programs

The agency provides services for older adults age 55 and older in Los Angeles county. Services include case/care management for frail elderly, an information line for seniors and PACE programs. Age restrictions apply. Case/care management services and information sources services are provided countywide. The Program of All Inclusive Care for the Elderly (PACE) is a Medicare and Medi-Cal program that provides comprehensive medical and social services for frail older adults to allow them to remain in their own home as an alternative to entering into a care facility. To qualify, an older adult must be certified by the state to need a nursing home level of care.

The program includes primary medical and specialty care, prescription medications, basic dental care, adult day health care (ADHC) services, and hospitalization as needed. Staff can assess and route calls for Medicare or health information, or make an appointment for primary care service at one of the agency's clinic locations.
