Adult Day Programs
Provider: SANTA CLARITA VALLEY SENIOR CENTER
The agency provides services for older adults age 60 and older and for disabled adults, including those with visual impairments, age 18 and older, who live in the Santa Clarita Valley and select surrounding areas. Services include adult day care, benefits assistance, blood pressure screening, caregiver support groups, case/care management, certificates/forms assistance, friendly visitation, geriatric counseling, glaucoma screening, home rehabilitation/repair services, legal counseling, mutual support groups, nutrition assessments and prescriptions, recreational activities, respite programs, senior community service employment programs, tax information, telephone reassurance, and volunteer opportunities. Age and geographic restrictions apply.
Adult day program is available for older adults who are frail or who have Alzheimer's disease and/or who have had a stroke. The program also provides respite care for their caregivers.
The Family Caregiver Support Program (FCSP) provides support to unpaid caregivers caring for frail, elderly relatives, and to older adults providing care for younger family members. The program provides access to support services including respite care, caregiver training, support groups, and counseling. Health services include occasional health fairs and regular screenings for glaucoma, blood pressure, nutritional health, and other geriatric-related issues. Prescription assistance and geriatric counseling are also provided.
Home rehabilitation/repair services are provided for low to moderate income residents of the Santa Clarita Valley and the Val Verde/Castaic area, who own and live in a single family dwelling. Assistance is provided with minor plumbing and electrical repairs; interior and exterior painting; and installation of doors, windows, screens, bathroom fixtures, entry and deadbolt locks, weather stripping and smoke detectors. To qualify, people must meet the low or moderate HUD income guidelines, live in the service area, and provide verification that they own and occupy their home. Work may be performed for renters under certain circumstances.
The Supportive Services Program (SSP) and Linkages program provide case management services to assess the physical, psychological, and social needs of older adults age 60 and older and persons with disabilities to allow more frail seniors to stay in their homes as long as appropriately possible and prevent premature or inappropriate institutionalization.
The Traditional Legal Assistance Program (TLAP) provides legal assistance including general counseling, benefits assistance, certificates/forms assistance, naturalization assistance, and tax information provided by visiting attorneys from Bet Tzedek once per month. Forms assistance is provided during tax season only.
The telephone reassurance program provides phone calls five days per week for older adults age 60 and older who are isolated or homebound. If a volunteer is unable to contact the older adult within a reasonable amount of time (usually 1 day), program staff call a family member or close friend.
