Health Insurance/Dental Coverage (Frail Elderly)

The agency provides health insurance for older adults and frail elderly adults age 65 and older in Los Angeles who qualify for Medicare Advantage programs. Services include administrative entities, and health insurance/dental coverage. Age restrictions apply. There are no geographic restrictions.
This is the administrative office which provides no direct services; it coordinates, administers, and houses a variety of health and social service programs.
To be eligible for the program persons must be Medicare recipients who qualify for Medicare Advantage programs (they must be covered under Medicare Parts A and B). People with End Stage Renal Disease (ESRD) are not eligible for coverage.
Services include medical and dental coverage; pharmacy services; vision services; long term comprehensive case management which may include assessment of a person's needs; the purchase of adult day care services and counseling; information about and referral to a variety of resources designed for seniors; and medical transportation.
The program provides medical transportation for SCAN members. Cabs and lift equipped vans are available. Eligible people may use the service for any medical or dental appointments. The program does not provide escort service; people who require an escort make their own arrangements.

People phone member services 24 hours in advance to reserve transportation, and must be ready 20 to 30 minutes before the scheduled pick-up time.

Fees
There are fees for some services; call for information. Depending on member coverage, a small co-payment may be charged for transportation.
