Wraparound Facilitation/Community Support
Provider: TESSIE CLEVELAND COMMUNITY SERVICE CORP.
The agency provides mental health services for youth age 18 and younger, transitional youth age 15 to 25, and seniors and their families, in South Los Angeles. Services include adolescent, family, group, and individual counseling; behavior modification; in-person crisis intervention; mental health screening; psychiatric day treatment; psychiatric medication monitoring and Wraparound services. Age restrictions apply; there are no geographic restrictions.
Staff sometimes make home-visits or provide agency transportation, and will work with each family to help ensure that service is accessible.
A family-centered approach to treatment starts with a comprehensive diagnostic screening and evaluation. Staff will consult with schools as needed to develop an individualized behavior intervention plan for children and youth with problems in school; harmful or unmanageable behavior; sibling conflicts; emotional, physical, or sexual abuse; social withdrawal; peer problems; hyperactivity or ADHD; adjustment to parental separation or new placement; coping with death; or depression. Staff will also work with parents to develop effective parenting skills.
The Wraparound facilitation/community support program is provided for court-referred youth in probation or in DCFS child abuse or foster care programs. Wraparound is conducted by a multi-displinary team consisting of a social worker case manager, a therapist and a parent "Partner". Sessions are conducted in the home and may involve just the child or the whole family as needed. The program accepts youth from age 8 to 18, and may continue for one year or longer. The service area is L A County SPAs 6, and is under the auspices of Special Service for Groups.
Additional services for clients may include case management, socialization, and community skills intervention. Referrals to additional community resources are available for all clients.

Application Procedure
Walk in for most services. Service is provided on a first-come, first-served basis; only the first 9 individuals will be seen each day.
Adults age 18 and over must show ID, birth certificate, social security card and Medi-Cal/insurance cards. A list of current medications and dosages must also be provided.
Youth under age 18 must have a caregiver present. The caregiver must show ID, his/her and child's social security cards, child's Medi-Cal/insurance card, child's birth certificate, legal guardianship paperwork and a minute order. A list of current medications and dosages must also be provided.
Transitional age youth must live in SPA 6, 7, and 8 and meet criteria for our Full Service Partnership program.

Fees
Fees are set on a sliding scale that generally ranges from $0 to $25; Medi-Cal, Medicare and private insurance are accepted.
