Housing Related Coordinated Entry

The agency participates in the Coordinated Entry System (CES) in Los Angeles County. CES provides a centralized system of individual assessment and matching to available housing resources and programs based on a number of factors. There are a variety of programs and resources available based on need, eligibility, and capacity. CES is targeted to people that are experiencing homelessness or at-risk of being homeless. A centralized software called HMIS is used to track case information.
CES for Single Adults is for individuals over the age of 18 who are experiencing homelessness in Los Angeles County. Adults may have households that are composed of themselves alone, married or unmarried partners, or adult children over the age of 18. (All individuals in the household are considered adults.)
CES for Youth (aka Next Step or Youth CES) is for youth 18 - 24 experiencing homelessness or housing instability in Los Angeles County.
CES for Families is for families with minor (under 18) children and certain pregnant women (2nd-3rd trimester or high risk). Families must be currently experiencing homelessness or at risk of becoming homeless. CES for Families services are coordinated through the Family Solution Centers (see separate entry).
Housing navigation is available for homeless participants who do not yet have a primary case manager, and do not qualify for a permanent housing program with case management services included. Participants are provided with a range of supportive services and linkages. The program must work in collaboration with the Coordinated Entry System (CES) for Singles.
Problem-Solving is a person-centered, short-term housing intervention that seeks to assist households in maintaining their current housing or identifying an immediate and safe housing alternative within their own social network. This strategy is a strengths-based approach that utilizes conversation and empowerment methods to help resolve the household's housing crisis, or quickly connect them to existing emergency or crisis housing services, by working alongside them in an empowering manner.
The program also provides general case/care management targeted to housing issues.
