Case/Care Management (African American Community, Pregnant Women)

Provider: CITY OF LONG BEACH DEPARTMENT OF HEALTH AND HUMAN SERVICES - MILLER FAMILY HEALTH EDUCATION CENTER

The program provides case management for pregnant African American women or parenting African American women who have children under the age of 2 years old. Services are restricted to residents of the city of Long Beach. Geographic restrictions apply.

The goal of the program is to promote infant health and reduce infant mortality among African American women. The program provides assistance obtaining prenatal care, free or low cost health/dental care, WIC program/nutrition education, housing, infant care and immunizations, domestic violence awareness, substance abuse prevention and intervention, tobacco education to help with smoking cessation, Afro centric parenting, workshops, and social support and empowerment.
