Contract Clinics

This community clinic provides health services for all ages. Services include child health and disability prevention exams, community clinics, contract clinics, general medical care and pediatrics. The clinic also provides Child Health and Disability Prevention (CHDP) exams. The clinic serves families living, primarily, in the Glendale, Burbank, La Canada, North Hollywood, Pasadena, areas; however, there are no geographic restrictions.

Contract clinic services include outpatient health services for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary general medical care. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients.

CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.

The clinic is a My Health LA Community Partner Clinic and provides application and enrollment in the My Health LA plan. My Health LA covers low income individuals (age 6 to adults) in Los Angeles County that do not have health insurance and cannot qualify for Medi-Cal.
