Homeless Permanent Supportive Housing
Provider: HOMES FOR LIFE FOUNDATION
The foundation provides housing services for adults with chronic mental illness/emotional disabilities or illnesses who live in Los Angeles County. Services include homeless permanent supportive housing. There are no geographic restrictions.
Types of housing provided are transition-age-youth housing, transitional adult housing, state licensed group homes, supportive apartments and semi-independent group homes.
Staff provide individual and group mental health sessions for adults with mental illness. Rehabilitation and skills development services are provided and consist of medication and symptom management, communication and relationship skills, social problem solving, community involvement and use of leisure time, meal preparation skills, money management and consumer awareness and vocational training and job seeking skills. Case managers provide consultation and linkage to community resources.
