State/Local Health Insurance Information/Counseling (Youth)

The agency provides health insurance services to families who have children, age zero to five, who are without health insurance and are not covered by any other social or private insurance programs. Services include state/local health insurance information/counseling. The agency serves SPA 4 (Downtown, Hollywood and Westlake area); there are geographic restrictions.

The agency is authorized to conduct enrollment in the Healthy Kids insurance program. The insurance program is provided by the Children's Health Initiative of Greater Los Angeles, and LA Care Health Plan (see the separate entries for details). The agency conducts workshops for interested parents and help to complete the applications, which are forwarded to the insurers.

Application Procedure
Call the agency's main number to find out about enrollment in Healthy Kids. Documentation requires proof of family income, including previous year's income tax form; proof of residence, such as a utility bill, rent receipt or driver's license. The person applying may be the parent, step parent, guardian or relative care giver.

There are no fees to enroll; premiums are based on the number of children enrolled.
