The Center is a private clinic with two locations for people of all ages who reside, primarily, in the Pomona and Inland Valleys. Services include childhood immunizations, a diabetes management clinic, dental care, pediatric dentistry, family and community medicine, optometry, osteopathy, podiatry/foot care, a pharmacy with compounding services, a travel medicine program that provides travel immunizations, and a women's health center. Services at the satellite location in Rancho Cucamonga are family medicine, diabetes management, osteopathy and podiatry. There are no geographic restrictions.

The family medicine focus of the clinic includes general medical care for the whole family, standard physical examinations, childhood immunizations, flu shots, and women's services.

The dental clinic serves children and adults for preventative and restorative dental care including x-rays, oral surgery, endodontics and periodontics. Dental care is available only at the main Pomona location. For endodontic and periodontic care, individuals must have a referral or must enroll as a new patient.

The Western Diabetes Institute (WDI) provides comprehensive and ongoing care of chronic diabetes in a multidisciplinary approach that combines the services of endocrinologists, pharmacists, podiatrists, optometrists and dentists. Treatments are personalized for each patient and follow through the treatment of severe diabetes complications such a kidney disorders.

Optometric services include eye examinations for adults and children, testing for eye conditions such as macular degeneration, and glaucoma and fitting for eyeglasses. Frames are available for purchase.

Podiatry/foot care services work with patients of all ages for foot and ankle related injuries such as fractures and conditions such as bunions, hammertoes, ingrown toenails, and diabetes-related conditions.

Pharmacy services are available on-site for patients and for the general public. The PCC pharmacy is a compounding pharmacy that can provide a prescription in alternative forms such a liquid, for people who cannot tolerate pill or tablets. The Compounding Pharmacy also prepares alternative medication formulas for animals and pets.

The Travel Health clinic provides consultation and immunizations for people traveling outside the United States. An appointment at least two weeks before travel date is required.

Western University is a teaching facility that prepares Doctors of Osteopathy. The DO is an MD with additional training in osteopathic manipulative medicine. Osteopathic treatment is available at both Western PCC locations. Students assist doctors in family medicine and diabetes management clinics. Dental students, medical students, and pharmacy students, working under professional supervision can provide services at lower cost to low-income patients.
