Case/Care Management (Older Adults)

The agency participates in the Older Adult Field Capable Clinical Services (FCCS) program through funding from the Department of Mental Health. FCCS services are geared for adults, age 60 and older, who are experiencing serious and persistent mental illness. Services include Mental Health Evaluation for Older Adults, Psychiatric Medication Monitoring, and Case/Care Management for Older Adults.

Through Field Capable Clinical Services, multi-disciplinary teams of mental health professionals can make home visits to provide support and advocacy for the elderly mentally ill. The program can provide linkages with community services such as social connections, food program referrals and medical referrals.
