The UCLA Fit for Healthy Weight Program provides weight management programs for obese children and adolescents and their families. Services include weight management.
The program provides clinical services, health education, and resources related to the prevention and treatment of childhood obesity. The Clinic provides a multidisciplinary team which includes general pediatrician, medical and surgical subspecialists, dietitian and psychologist/psychiatrist. The team works together to provide an individualized treatment plan to achieve a healthy weight. The program is design to support the entire family.

Application Procedure
A referral from the family"s primary care provider is required. The physician fills out the referral form and faxes it along with lab results to the UCLA Fit for Healthy Weight Program fax number listed. Also can email questions about the program to fitprogram@mednet.ucla.edu.
Fees
Insurance may be accepted by PPO, Medi-Cal, some cases the California Children's Services. The clinic also accepts self pay.
