Nutrition Education (Youth)

The center provides health services for infants, children, adolescents and young adults in Los Angeles County. Services include adolescent medicine, allergy testing, child health and disability prevention exams, health/disability related counseling, hospital based outpatient services, nutrition education, pain management, pediatrics, physician referrals, referrals to physicians accepting medicaid, and well baby care. There are no geographic restrictions.
CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level.
Specialty health services include Allergy/Immunology, Bone Marrow Transplantation, Cardiology, Child Development, Endocrinology, Gastroenterology, Genetics, Heart Transplantation, Hematology/Oncology, Kidney Transplantation, Liver Transplantation, Nephrology/Dialysis, Neurology, and Pain Management. Physician referral services are made to health center doctors and include payment information for patients receiving Medi-Cal and Medicare.

Application Procedure
Call to apply; a first-time assessment is required. There may be a waiting list. If the waiting list has been closed, the center can refer the client to another appropriate medical service center.
Fees
There are fixed fees for services; Medi-Cal, Medicare and private insurance are accepted. Payment is requested at the time of service if a person does not have insurance.
The Well-Child-Care program (CHDP) may pay for the visit and immunizations for patients who are financially eligible.
