Health Insurance Information/Counseling
Provider: L A CARE HEALTH PLAN - COMMUNITY RESOURCE CENTER - EAST LOS ANGELES
The organization administers health insurance programs for children and adults who live in Los Angeles County. L.A. Care Health Plan participates in Medi-Cal, Medicare Plus, Covered California (L.A. Care Covered), and PASC-SEIU. There are no geographic restrictions.

Medi-Cal covers low-income adults and children. Members may choose to receive care from L A Care, Anthem Blue Cross, Care 1st Health Plan, or Kaiser Permanente. Program benefits include dental care, doctor visits, hospital and emergency room care, immunizations, mental health services, prescriptions, substance abuse services, and vision care.
MediCare Plus is an HMO Dual Eligible Special Needs Plan (HMO D-SNP) for individuals who are dual eligible for Medicare and Medi-Cal.

Documentation requires proof of family income, including previous year's income tax form; proof of residence, such as a utility bill, rent receipt or driver's license.
