Case/Care Management (Frail Elderly)

Provider: ALHAMBRA CASE MANAGEMENT PROJECT

The program provides a variety of services for adults 60 years and older who live in Alhambra. Services include case/care management and telephone reassurance. Services are restricted to Alhambra residents.

Case Management services are provided to seniors living in Alhambra to help them remain living in their homes for as long as it is safely possible. Case Workers assess clients to determine their needs, develop a care plan for services, coordinate services, and maintain monthly contact with clients. Case Workers can also assist clients with advocacy, paperwork, transportation services, legal issues and more.

Telephone reassurance provides weekly phone contact to seniors to check on their well-being and provide support. Calls are made by senior volunteers who provide friendly contact with the outside world. If the client is not able to be reached staff can contact family members or emergency contact to let them know client has not been answering their telephone.
