Case/Care Management

Provider: SPECIAL SERVICE FOR GROUPS - OLDER ADULTS PROGRAM

The agency provides services for low-income older adults, age 60 and older and for adults with disabilities. Services include case/care management, caregiver support groups, services for intergenerational caregivers, and specialized information and referral to health and community services. The case management program is also available for disabled adults. There are no geographic restrictions.

ICM is a care management program for older adults and disabled adults who have limitations.

The agency also participates in the Older Adult Full Service Partnership (FSP)and Field Capable Clinical Services (FCCS) program for older adults through funding from the Department of Mental Health. FSP services are geared for adults, age 60 and older, who are experiencing serious and persistent mental illness. Through Field Capable Clinical Services, multidisicplinary teams of mental health professionals can make home visits to provide support and advocacy for the frail elderly mentally ill.

The Supportive Services Program (SSP) and Linkages program provide case management services for older adults age 60 and older and persons with disabilities to allow them to stay in their homes as long as appropriately possible and prevent premature or inappropriate institutionalization.
