State/Local Health Insurance Information/Counseling
Provider: EMANATE HEALTH - QUEEN OF THE VALLEY HOSPITAL
The hospital provides health insurance information for families and individuals in the San Gabriel Valley area. Services include state/local health insurance information/counseling through the Get Enrollment Moving Program. There are no geographic restrictions.
The hospital is authorized to conduct enrollment in the Healthy Kids insurance program for families who have children, age zero to five, who are without health insurance, and are not covered by any other social or private insurance programs. The insurance program is provided by the Children's Health Initiative of Greater Los Angeles, and L A Care Health Plan (see the separate entries for details). The agency conducts workshops for interested parents and help to complete the applications, which are forwarded to the insurers.
Project staff will screen individuals and families for Medi-Cal and other health benefit and insurance plans, and assist with applications.
