Medically Monitored Physical Fitness

  The agency provides adult day health care services for frail older adults, age 55 and older, and for developmentally disabled adults, age 18 or older who live within the Los Angeles area. Services include adult day health programs, medically monitored physical fitness and recreational therapy. There are geographic restrictions.

    Services include adult day health care that provides cultural and recreational activities, exercise programs, health screening, individual and group counseling, meals, nutritional counseling, occupational therapy, physical therapy, psychiatric evaluations, psychological evaluations, social service referrals, speech therapy, and free transportation.

     The program's goal is to help prevent premature institutionalization of its clients. A participant will stay at the center between four and five hours a day in order to provide an atmosphere for stimulating activities and to provide respite care for adult care givers at the client's home. Psychiatric services are offered once a week. Basic health needs, like blood pressure and blood sugar levels, are also monitored on an as needed basis.

    The agency will provide free transportation to any individual in the program as long as they live within an hour's driving distance from the facility. If a client lives beyond this range, arrangements can be made to continue transportation if the client provides a service area waiver and a note from a physician. The agency will make psychiatric referrals on a limited basis.
