The agency provides services for older adults age 55 and older in Los Angeles county. Services include case/care management for frail elderly, an information line for seniors and PACE programs. Age restrictions apply. Case/care management services and information sources services are provided countywide. PACE is a managed care program that provides medical, social, nutritional and rehabilitative services to older adults. The program allows older adults who are frail and meet nursing home eligibility criteria to live in their own homes. The program includes primary medical and specialty care, prescription medications, basic dental care, adult day health care (ADHC) services, and hospitalization as needed. The central intake number offers an option to receive information specific to seniors and older adult caregivers. Staff can assess and route calls for Medicare or health information, or make an appointment for primary care service at one of the agency's clinic locations.
