Veteran Homes
Provider: CALIFORNIA STATE DEPARTMENT OF VETERANS AFFAIRS - VETERANS HOMES OF CALIFORNIA - CHULA VISTA
The program provides out-of-home-care for veterans age 55 and older, as well as disabled veterans and homeless veterans in need of long term care, who are current residents of the State of California. Services include veterans homes. There are no geographic restrictions; however, individuals must be a current resident of California.
The program provides a range of levels of care, depending on the facility, that include Domiciliary or Independent Living, Assisted Living through the Residential Care for the Elderly Program (RCFE), Intermediate Nursing Care, and Skilled Nursing Care. The Barstow and Yountville VHC's currently have no waiting list for Domiciliary admissions.
Ambulatory Care Clinics serves residents who live in the RCFE or the Domiciliary neighborhoods. These clinics provide comprehensive multidisciplinary assessments, well as ongoing primary care.
Admission requirements include the following:
1.Served on active duty (a minimum of 90 days) for other than training purposes, during wartime or peacetime.
2.Provide proof of military service by submitting a DD 214, or obtaining proof through the U.S. Department of Defense (DOD) or the United States Department of Veterans Affairs (USDVA).
3.Discharged or released from active duty honorably or under honorable conditions.
4.California resident at the time of application for admission.
5.Test negative for Tuberculosis (TB).
6.Not require more care and supervision than provided at the Veterans Home of choice.
7.Not require acute hospitalization at the time of application.
8.Not have a primary need for acute psychiatric care.
9.Not have a past history of violence, mental illness, or a criminal record.
10.Not be under the influence of alcohol or illegal substances.
11.Be enrolled in a qualified federal, state, or private health insurance plan, or have an application for such a plan pending.
Note: Medal of Honor recipients, Ex-POW's and wartime veterans are given priority for admission over peacetime veterans.
