Certificates/Forms Assistance

The program provides advocacy for people in Los Angeles County to help obtain benefits and services to which they are entitled. Service includes assistance with health insurance benefits applications, including Medi-Cal; and eye examinations by the UCLA Jules Stein Eye Institute's mobile clinic.

Community Health Councils, Inc. provides the health insurance assistance; the agency assists adults and families with children, process and apply for Medi-Cal or other insurance options. There are no geographic restrictions.

Eye exams are provided periodically on-site by the mobile clinic.
