Services include in home medical care for patients unable to present to a doctors office. The agency provides in home care to the at the risk population (unable or difficulty presenting to their doctor's office due to social or medical reasons, often with multiple complex medical issues) at no cost. Each person is paired with a care coordinator, provider, and physician at intake. Between visits the person utilizing the services can contact their care coordinator to address specific needs that may arise.
