Case/Care Management (At Risk Adults, Pregnant Women)

The agency provides child abuse services for at risk families with children age 5 or younger in the Metro North area of SPA 4. Service includes child abuse prevention. Services are restricted to families within SPA 4.
The agency provides child abuse prevention and intervention services for at risk families with children ags 0-18 in the Metro North area of SPA 4 and the in the Wateridge offices of SPA 6. Services are restricted to families within SPA 4 and 6.
The Child Abuse Prevention, Intervention and Treatment (CAPIT) program is available for all families residing in Supervisorial District 1 (part of SPA 4, 6, 7 and 3). This program support children and families with active case management services, basic need support, an Evidenced Based Nurturing Parenting program, early family literacy and child social skills support.
The Family Preservation Program support families who are reuniting with their children or who's home situation poses potential risks. In-Home services are provided only to families who are referred through the Department of Children and Family Services (DCFS).
The agency's Partnership for Families in an inter agency collaborative in conjunction with El Centro del Pueblo, KYCC, Plaza Community Services and St. Anne's offering in-home services to pregnant mothers and parenting teens as well as to DCFS referred families with children 5 and under in SPA 4. Direct services include case management, crisis counseling, parenting education, information and referrals to community resources such as food, clothing, shelter, transportation, etc., in-home visitation, crisis intervention, family advocacy, support groups, and substance abuse, domestic violence and mental health services.
