Provider: YOUNG AND HEALTHY
The program provides access to health services for uninsured or underinsured low-income families in Altadena, Pasadena, South Pasadena, and Sierra Madre. Services are targeted to children age 0 to age 18. Services include adolescent/youth counseling; case/care management; child guidance; dental care; dental screening; general laboratory tests; health education on dental hygiene; pediatrics; prescription medication services; and mobile dental care. Services are restricted to children within the boundaries of the Pasadena Unified School District and the city of South Pasadena.
Referrals of eligible low-income youth may come from the PUSD, shelters, the Pasadena Public Health Department or community based organizations. To qualify, the youth must not be currently enrolled in Medi-Cal or other insurance program, and family income must meet poverty guidelines set by the program. The program will serve undocumented families for certain services.
Case management services provide an assessment and referral to needed dental, medical, and mental health providers, who are volunteer professionals in the Pasadena area. The youth are seen in the private offices of the providers.
Health education for dental hygiene is conducted by volunteers in the schools. Dental referrals and screening is done through the USC Dental School mobile dental care clinic. Prenatal mothers and parents of preschoolers may also be seen through the USC Dental School.
