State/Local Health Insurance Information/Counseling

The agency is contracted to provide health insurance information for families who have children, age zero to five, who are without health insurance and are not covered by any other social or private insurance programs. Service includes state/local health insurance information/counseling. The insurance program is provided by the Children's Health Initiative of Greater Los Angeles, and LA Care Health Plan (see the separate entries for details). The agency conducts workshops for interested parents and help to complete the applications, which are forwarded to the insurers. The agency serves families in SPA 4, which is the metro area of Los Angeles and includes zip codes: 90004, 90005, 90006, 90012, 90013, 90014, 90015, 90017, 90019, 90020, 90021, 90023, 90026, 90027, 90028, 90029, 90031, 90032, 90033, 90035, 90036, 90038, 90039, 90041, 90042, 90046, 90048, 90057, 90063, 90065, 90068, and 90069. APPLICATION PROCEDURE Call the agency's insurance enrollment hotline number to find out about enrollment in Healthy kids. Documentation requires proof of family income, including previous year's income tax form; proof of residence, such as a utility bill, rent receipt or driver's license. The person applying may be the parent, step parent, guardian or relative care giver. Call the agency verify exact application procedure.
