Medicare Information/Counseling
Provider: CITY OF AZUSA COMMUNITY RESOURCES DEPARTMENT - AZUSA SENIOR CENTER
The center provides a variety of services for adults 50 years and older who live in the Azusa area. Services include information and referral; case management, blood pressure, social activities and trips, Medicare counseling, and recreational/leisure/arts instruction classes. People who are age 60 and older are eligible for case management services. Most services are restricted to residents of Azusa.
Case management services are provided through the YWCA. Workers to assess the needs of frail seniors and develop, implement, and coordinate a service plan that can help support seniors in their homes. The service can continue as long as it is helpful.
