Adult Day Health Programs (Asian Community)
Provider: COMMUNITY BASED ADULT SERVICE CENTER
The center provides adult day health care services for frail elderly, and for disabled adults, age 18 and older. The services are targeted, but not restricted, to the Chinese speaking community and those whose physical, mental and developmental challenges make independent living difficult, and who may be at risk for institutionalization. Transportation services are provided only to those who live within the primary service area, San Gabriel Valley, however, there are no geographic restrictions for enrollment.
The program provides supportive services such as case management for social services; nursing services; nutrition services, including balanced meals and nutritional counseling; psychiatric/psychological counseling; physical, occupational and speech therapy; and social and recreational activities. Participants may enroll for one to five days per week. Transportation is provided for clients of the program.

Fees
Fees are set on a sliding scale that ranges from $0 to $80 per day. Medi-Cal and private pay are accepted.
