Terminal Illness Counseling
Provider: MINORITY AIDS PROJECT
The agency provides HIV/AIDS services for people of all ages in Los Angeles. Services include case management, HIV screening and counseling, disease/disability information and outreach services. Some services are targeted, but not restricted, to African Americans and Hispanics/Latinos. There are no geographic restrictions.
Early intervention is provided through a Comprehensive and Culturally Relevant Continuum of Care (CCRCC) program which offers a broad range of services in response to the social, economic and medical challenges facing the community. Comprehensive services may include: treatment adherence, mental health and support group services, case management, assistance with obtaining public benefits, substance abuse screenings and treatment, food, clothing, transportation, health education and risk reduction, transgender prevention services, HIV counseling and testing, HIV intervention and prevention services, housing assistance, and in-home nursing and attendant care case management.
Linkages to Care is a short term outreach program for individuals who are newly diagnosed with HIV or who have been out of care for seven or more months. Staff visit clients in their homes or other community locations to identify obstacles and develop a plan for care. Staff will link clients with doctors, clinics or other health care providers and accompany them at appointments.
Educational services are available to the public including, AIDS 101 classes, staff training in HIV/AIDS issues for social services agencies, speakers for public events, information at health fairs and community festivals, and conference workshops.
The agency also accepts monetary donations as well as donations of food, clothing, automobiles, boats and motorcycles.
