Health/Disability Related Support Groups (AIDS)

Case/care management services provide integration of services for a client, based on an individualized service plan. Clients may receive dental care; emergency food assistance from the agency's food pantry; health education, and nutrition counseling; work services; and benefits assistance, as needed. The food pantry, NOLP, offers special diet food that meet the dietary needs of people with HIV/AIDS. The program also collaborates with other HIV/AIDS programs throughout the county to provide this special pantry service for people living with HIV/AIDS. Mental health services include crisis intervention, assessment counseling, substance abuse prevention assistance, and specialized referrals for additional services. Significant others of clients or people living with HIV/AIDS may also apply for support and counseling; support groups are available. Adults and teens who have HIV/AIDS who are involved with alcohol or drug abuse or who have a problem with any addictive behavior, including eating or using sex addictively, may be involved in individual counseling and support groups to deal with the addiction. Mutual peer support groups are provided for people with HIV/AIDS and their families. Special groups for women are also available.
The agency also offers the HIV/AIDS PrEP program that provides pre-exposure treatment for people who have high-risk HIV/AIDS exposure. PrEP includes a daily medication to reduce the risk of HIV infection.
The center enrolls patients for the AIDS Drug Assistance Program (ADAP), which provides FDA-approved HIV-related drugs at little or no cost to those who are eligible; call for more information.
Home health services include medical case management, psychosocial services and attendant care in the home. Other services available to registered members of APLA include transportation to and from doctor's appointments, bus passes, and treatment advocacy.
Clientline provides resource information, referrals, and telephone case management for clients, significant others, family members and caregivers.
Information and education is provided through a speakers bureau, community presentations, and written materials. Training for professional health and mental health workers is also available. The program provides advocacy and builds awareness of the impact of HIV/AIDS at all levels of government. The agency advocates for increased funding for HIV/AIDS services and policy to improve the lives of those living with HIV. The program also connects individuals with their elected officials and provides information on public policy issues. LANGUAGES OTHER THAN ENGLISH Spanish; Translation services for other languages may be available with advance notice.
